Traumatic brain injury in this page is related to moderate and severe brain injury, not what we commonly call 'concussion'.
Typically Concussion is associated with a Glasgow Coma Scale of 13 - 15.
Moderate Brain injury GCS:  9 - 12
Severe Brain injury GCS:  8 or less

Managing Traumatic Brain Injury (TBI) in a rural setting really involves 2 goals: 
prevent hypotension (systolic < 90) and hypoxia (PaO2 < 60)
This involves understanding the pathophysiology of cerebral blood flow.
